To realign proper incentives and reduce preventable costs, in 2015, Center for Medicaid and Medicare declared a goal to change reimbursement model from 90% fee-for-service to 90% value-based. Whether it’s capitation, bundled payments, or readmissions penalties, there is one thing we know for certain - avoidable mistakes are no longer tolerable. Meanwhile, the one place that health care providers have yet to make a meaningful impact is also the one place that offer the greatest opportunity for improvement - engaging patients outside of the boundaries of the hospital.
