LivWell Health is a cloud-based software-as-a-service (SaaS) company. We build solutions that:
1.) Bridge the gap between post-acute care teams (Social Workers, Care Coordinators/Navigators, Discharge Nurses and Primary Care Physicians) and home and community based service providers (HCBS) with a comprehensive service and referral management platform.
2.) Expand the capabilities of Long Term Care (LTC) providers such as Senior Care and Retirement Living, Adult Day Centers, Skilled Nursing, Respite and Hospice Care provicers to more effectively communicate with residents, caregivers and family members through digital signage and resident/family portals.
