Kaizen Health is a technology solution; the hub that connects patients, healthcare providers, transportation providers and payers to increase access to healthcare and eliminate the technology barriers from the patient.
Care managers will use Kaizen Health’s platform to schedule a ride for a patient. This can be to discharge a patient or schedule a ride for a future appointment. The patient will receive communication via text or phone call to remind them of upcoming appointments and the arrival of their transportation. We partner with the ride share community and NEMT providers to provide the transportation. Our platform tracks the patient to ensure their safety, while eliminating fraudulent abuse of transportation programs.
