Signallamp Health was founded on the clear understanding that human interaction is essential to improving the management of chronic health conditions. While we leverage world class technology to empower our Nurse Assistants with relevant and real-time data, the heart of our approach is rooted in time-tested, evidence-based care management and behavioral economics. Rate compression and better patient outcomes are usually at odds; the challenge is that too few resources are available to add staffing and develop the IT framework to support an dedicated effort that can drive meaningful results. This has created an over-reliance on automation, self-service and less-meaningful patient contact, which has shown short-term improvements at best, but more often has been ineffective. Our approach had to be different. We created a way to target and interact with patients, seamlessly integrate with a practice's EMR, and drive practice revenue with absolutely no upfront investment or disruptions to workflow. The Signallamp Health model leverages difficult to capture reimbursements, performance/risk-based contracting, and missed practice revenue to deliver improved patient care.
